Operational Briefing: Lessons from Weston Bay Man Overboard

Immediate Operational Checks:

Following the tragic double man overboard incident from the fishing vessel Weston Bay on 22 May 2024, which resulted in one fatality, it is crucial for all officers to immediately review and reinforce critical safety protocols, especially concerning deck operations. Ensure that comprehensive risk assessments are in place and actively followed for all tasks involving working near the vessel’s side or with running gear, such as shooting pots. Verify that all crew members are issued with and correctly wear appropriate personal flotation devices (PFDs) when on deck, and that these PFDs are properly fitted to prevent slippage. Conduct immediate drills to confirm the efficiency and effectiveness of man overboard recovery systems and procedures, ensuring all crew are proficient in their use and understand their roles. Emphasize clear communication protocols during all deck operations to minimize the risk of accidents.
